• Technical: Problems related to the technical aspects of performing a procedure, such as inadvertent injury to an organ.
• Judgment: Problems related to decision making, such as delays diagnosis or treatment, or unsafe lack of knowledge.
• Equipment, supplies, or devices: Problems related to availability or function of equipment, supplies, or devices, such as unclean instruments, broken devices, or unavailable critical supplies.
• Policies, process, or environment: Problems related to an unsafe environment or a failed or missing policy or process, such as delivery of the wrong blood products to the operating room.
• Operations: Problems related to insufficient staffing, inadequate bed or resource availability, etc.
• Professionalism: Problems related to conduct, failure to meet responsibilities, or interactions with others, such as the use of demeaning or intimidating language.
• No harm or additional treatment: In addition to events that reached the patient, but did not cause harm, please include potential events (unsafe conditions without an event) and near misses (event occurred, but did not reach patient)
• Adverse event – treatment required: Event necessitated additional intervention or increased length of stay, but caused no other injury. Includes treatment provided after discovery of the event, as well as anticipated future treatment directly related to the event.
• Adverse event – temporary harm: Physical or psychological injury, but not likely to be permanent.
• Adverse event – permanent harm: Lifelong physical or psychological injury, or increased susceptibility to disease.
